CVC Post- Hemodialysis Vital Signs: BP Temp PR RR Post- procedure Notes: Hemodialysis treatment completed and terminated. Treatment well-tolerated with net UF of ___ mL. BVP ____. CVC ports flushed with NS, clamped and capped with Tego. Patient asymptomatic post-treatment; denies complaints and discomfort. Treatment well-tolerated with net UF of ___ mL. BVP ____. Hemodialysis access decannulated. Hemostasis achieved on both access sites in less than 5 minutes. Gauzes applied. No bleeding noted on access sites.  Patient asymptomatic post-treatment. Educated to avoid putting pressure on access arm to promote further hemostasis. Post report given to primary RN
